Community Based Support offers a range of aged care services to help you stay independent, remain connected to your community, and continue living safely at home.

As an approved Support at Home provider, CBS delivers personalised care and assistance tailored to your needs. Our services are available in your home, in the community, and at our centres in southern Tasmania.

Our experienced team works with you to create a care plan that supports your choices and helps you live the life you choose.

How Support at Home works

The program offers tailored support based on individual needs, rather than fixed package levels. After an assessment, you will receive an approved support plan outlining the services available to you. These may include:

  • Clinical care: such as nursing care, occupational therapy and continence care.
  • Independence: such as help with showering, getting dressed, taking medications, transport or respite care.
  • Everyday living: such as cleaning, gardening, shopping or meal preparation.

Flexible and personalised support

Support at Home replaces previous programs to create a simpler, more adaptable system. Services are tailored to match your evolving needs, ensuring you receive the right support at the right time.

Whether you’re just starting to look into aged care options or you’re ready to take the next step, our resources are here to help. They offer clear, practical information to guide you through our services, pricing and what to expect, so you can make informed decisions with confidence.
