Bernadette’s story: Small changes making a big difference

A small home modification has made a world of difference for Bernadette, giving her the confidence to move safely around her home again.

Finding the right solution

As a new client, Bernadette met with her Coordinator, Delecia, to talk through what would help her stay safe at home. “Bernadette was really clear about wanting to feel confident moving in and out of her home,” Delecia said. “It was important we found something simple that would make a big impact for her.”

An Occupational Therapist later confirmed that ramps at both the front and back doors would reduce Bernadette’s risk of falls and make everyday movement easier. With that recommendation, Delecia supported Bernadette to apply for Assistive Technology & Home Modifications (AT‑HM) funding, a process that ensures clients can access the right equipment or home changes when they need them. Once approved, the CBS Home Modifications team scheduled the work.

A small change with a big impact

The ramps were recently installed and while the changes may seem small, the impact has been significant.

“I feel so much more confident getting in and out of my home now,” Bernadette said. “It’s given me back some of my independence and it’s made everyday life so much easier.”

Bernadette’s daughter, Tanya, has seen the difference too.

“Bernie is no longer fearful of coming out both front and back doors,” she said. “Her freedom and independence to move around freely has been improved greatly.”

Tanya also appreciated the care shown throughout the installation. “Paul and his team showed outstanding workmanship. He was very professional and got the work done as quick as he could. Paul was very respectful and kept us up to date with the job’s progress whilst working around our home. Bernie and I are very grateful.”

Working together

For Delecia, seeing the outcome has been rewarding. “It’s lovely to see how much confidence she’s gained,” she said. “Sometimes it’s the smallest modifications that make the biggest difference.”

From the first conversation through to the final installation, the process has been a true team effort. For Bernadette, the new ramps have made everyday life easier and given her greater confidence to continue living safely and independently at home.

What is AT-HM funding?

AT‑HM funding sits separately from a client’s Support at Home services. It’s designed specifically to cover equipment and home modifications, even for new clients like Bernadette who don’t have unspent package funds. This means people can access the changes they need to stay safe and independent at home, regardless of their care budget.
