For many of us, home is where we feel most ourselves. It is where routines are comfortable, where memories live, and where independence feels natural. But as we age, even the most familiar spaces can quietly become less safe. A step that was never a problem. A shower without a rail. A front path that is fine until it isn’t.
The good news is that Australia’s aged care system now has dedicated funding to change that, and many older Tasmanians may not yet realise what they are entitled to.
What is the AT-HM Scheme?
Under the Support at Home program, which replaced Home Care Packages in November 2025, the Australian Government introduced the Assistive Technology and Home Modifications (AT-HM) Scheme. This provides funding specifically for equipment and home changes that make everyday life safer. Critically, this funding is separate from a participant’s regular Support at Home quarterly budget, meaning you do not have to choose between modifications and your other care services.
The scheme covers a wide range of supports, from ramps, safety handrails and stair lifts through to walking aids, bathing equipment and toileting supports, with funding available up to $30,000. Funding is allocated across three tiers based on your assessed needs, from straightforward low-cost items right through to complex structural modifications.
Already on a Home Care Package? Your unspent funds could be the starting point.
Older Australians who were assessed or approved for a Home Care Package on or before 12 September 2024 transitioned automatically to Support at Home, retaining their existing package level and any unspent Home Care Package funds. For many, those accumulated funds represent a ready resource, and home modifications are among the most valuable ways to put them to work.
