Community Based Support offers a range of aged care services to help you remain connected to your community, stay at home and lead your best, independent life.

As a registered My Aged Care service provider, CBS provides all levels of Home Care Packages as well as a comprehensive range of services under the Commonwealth Home Support Program (CHSP) and Veterans Home Care (VHC) to people in their homes, in the community and in centres around Tasmania.

Our experienced and qualified staff ensure your needs are met by putting you in the middle of the picture to create your personalised care plan.

Home care packages

A Home Care Package delivers a coordinated package of care to meet your individual care needs.

The package provides an allocated amount of funds to allow for more comprehensive support and is tailored to meet individual needs.

There are four levels of home care packages.

  • Level 1 supports people with basic care needs;
  • Level 2 supports people with low-level care needs;
  • Level 3 support people with intermediate care needs;
  • Level 4 support people with high-level, often complex, care needs.

Home care packages are provided on a consumer directed care basis, this means you receive information about the package funding, including an individualised budget and monthly statements. This ensures that you, the client, are in the driving seat.

Getting a home care package

Don’t be confused or overwhelmed by the process. Contact our team who will provide expert guidance and assist you to contact My Aged Care. 

Otherwise, follow these steps:

  1. Contact My Aged Care to register and arrange a referral for assessment.
  1. My Aged Care will talk to you about your care needs and find out if a home care package is what you or your loved one needs.
  1. If you qualify, a referral will then be sent through to the Aged Care Assessment Team for a comprehensive assessment. The assessment will find what level of care is needed and approve for either a low care (level 1 or 2) or high care (level 3 or 4) Home Care Package.
  1. Once approved, you can start your research into finding an approved registered provider of Home Care Packages that will suit your needs.

If you don’t qualify, don’t despair. We can provide you with self-funded home support.

We know how important it is for you to stay in the place you are familiar with and to keep connected with your family and friends so our services are designed to make this possible.

  • Domestic assistance

    Domestic assistance, providing help with your household chores including cleaning and cooking.

  • Personal care

    Personal care includes assistance with showering, bathing and dressing; personal grooming such as hair styling or having a shave; applying topical creams and taking your medications among many other aspects of help.

  • Social support

    Social Support can aid you to access and stay connected in your community – visit the library, church or cinema, go shopping or attend a class to support fitness and agility – the choices are endless!

  • In-home respite

    Providing a break for carers. We know carers can find it difficult to balance looking after themselves as well as those they care for – this is where we can help by providing a break.

  • Allied health services

    We can arrange  allied health services such as physiotherapy and occupational therapy – they can help you bridge the gap between what you can do and what you want to be able to do. They can help with equipment, home modifications, skills training or developing adaptive techniques.

  • Community nursing

    Community Nursing services can be provided in your home by a qualified nurse and may be on a regular or occasional basis. Services can include wound care, blood pressures, catheter management, end of life care, drain management, continence, diabetes management, assistance with medications and other specialist nursing related activities.

  • Social hubs

    A fun way for people to keep in touch with friends in a safe and supportive environment. Activities are run at the hub location and there is also a scheduled program of activities out and about in the community.

  • Community transport

    Helps you to access your community, getting you where you need to be, when you need to be there.

  • Annual spring cleaning and garden maintenance

    For help with the bigger house cleaning jobs like windows, cleaning out gutters, cleaning out the oven and fridge or out in the garden with jobs like mowing the lawn, trimming trees and hedges, weeding etc. 

  • Home maintenance and modifications

    For basic things like changing tap washers, changing light globes and other minor household repairs all the way up to making it easier for you to access in and around your home by building ramps, putting rails in place and modifying bathrooms.

  • Personal and home safety

    CBS can support you to feel safe in your home and maintain your independence. Devices can include personal medical alarms, fall detectors, help buttons, smoke, gas and flood detectors and key safes
