We are looking for a highly motivated, experienced Care Facilitator who has a passion for delivering innovative and flexible support to clients and their carers. Community Based Support is a not for profit, Tasmanian organisation that supports people, who are frail aged or have disability, to live the life they choose while living in their home for as long as possible.
This position is full time with an attractive salary, including the option to participate in salary packaging and other benefits.
The successful person will have worked in case management and/or complex care coordination within the aged care sector, have excellent computer skills, and the ability to represent Community Based Support in a positive manner.
