An excellent opportunity exists to join our dedicated Disability Support Services team.
We are looking for a highly motivated, experienced Coordinator who has a passion for delivering innovative and flexible support to clients and their carers.

Community Based Support is a not for profit, Tasmanian organisation that supports people with disability or who are frail aged, to live the life they choose while living in their home for as long as possible.
This position is full time with an attractive salary, including the option to participate in salary packaging and other benefits. Initial engagement will be for 12 months however may be extended depending upon Government funding.
The successful person will:
have worked in complex care coordination within the Disability sector
have a high level of knowledge and experience with the National Disability Insurance Scheme (NDIS)
be focused on outcomes for participants
have excellent computer skills
have the ability to represent Community Based Support in a positive manner.
