Support To Live Independently
Community Based Support (CBS) is a Tasmanian organisation that offers a range of support services designed to help younger people with disability live the life they want, both at home and in their community.
CBS has been doing this since 1988 so, as you can imagine, with that sort of experience we are pretty confident that we can get it right.
The way we do this is by getting to know you, finding out what makes you tick, and then together we create a plan that is just for you.

There are lots of different options, it all just depends on your situation. We explain it a bit more below.
The Home and Community Care (HACC) program
The Tasmanian HACC program provides funding for basic community care services for people aged 64 years or less and are living with disability or mental illness. (For those who identify as Aboriginal and Torres Strait Islanders, the age group is 49 years and under). These services are for people who want to live independently, but need a bit of support to make it happen. It also helps to reduce the risk of having to go into supported accommodation before a person really wants to.
Support that can be provided under the HACC Program (HACC) include:
- Domestic Assistance, providing help with household chores;
- assistance with Personal Care such as showering and dressing;
- Social Support that assists people to access and stay connected in their communities;
- In Home Respite, providing a break for carers;
- basic Home Maintenance for things like changing tap washers, changing light globes and other minor household repairs;
- Home Modifications to make it easier for you to access in and around your home by building ramps, putting rails in place and modifying bathrooms;
- Allied Health services such as physiotherapy and occupational therapy;
- Community Nursing services can be provided in your home by a qualified nurse and may be on a regular or occasional basis. Services can include wound care, blood pressures, catheter management, end of life care, drain management, continence, diabetes management, assistance with medications and other specialist nursing related activities;
- the delivery of Meals to your home to assist you in staying healthy and well fed;
- centre based day care, or Social Clubs, provide a fun way for people to keep in touch with friends in a safe and supportive environment;
- Community Transport that helps to access your community, getting you where you need to be, when you need to be there;
- annual Spring Cleaning for the bigger cleaning jobs like windows, cleaning out gutters, cleaning out the oven and fridge etc.; and
- annual Garden Maintenance for jobs like mowing the lawn, trimming trees and hedges, weeding etc.
The Home and Community Care program provides low, entry level services.
To access this support, you will first need to register with Tasmanian Community Care Referral Service (TCCRS). The TCCRS is the first point of contact for all Tasmanians, aged 64 years and under, looking for disability or mental health support.
There are a number of ways this can be done:
1. Call the Tasmanian Community Care Referral Service (TCCRS) on Phone: 1300 769 699 or Fax: 1300 721 611.
2. Fill out this form and fax to TCCRS on 1300 721 611
3. Contact us at Community Based Support on 03 6208 6600 (South) or 03 6338 1889 (North/North-West) and speak to our disability support team who can help you over the phone, or you may prefer to email them here.
4. Come in to our office to see us and have a chat, face to face. It is important to note here that the Disability Support Services team are often out of the office speaking to people in their homes, so it might be worth contacting them first to make a time that is suitable to you.
The Tasmanian Community Care Referral Service (TCCRS) will talk to you and make an assessment about your eligibility for this type of funding. If you are eligible, the TCCRS will then send a referral on to an organisation, such as Community Based Support, that’s able to help you with your specific needs. If you want to, you can ask for your information to be passed on to Community Based Support (CBS) and we will do whatever we can to help you.

Do you need more support than the Home and Community Care program can provide?
If you find that the Home and Community Care (HACC) program services are not enough for you there are other options you can explore with Community Based Support.
The National Disability Insurance Scheme (NDIS) was first trialed in Australia in 2013. Community Based Support was involved in this trial and continues to be an authorised provider of services as the scheme is rolled out in full across the country. The NDIS has been introduced to support people living with disability to achieve their life goals. The support offered is for those who need more than just the basics. Individual Support Program (ISP) Packages are another option for people with a disability. This program, funded by the Tasmanian Government’s Disability Services, is very flexible and offers the options of choosing the support you need, in the way that you need it. It truly puts you in the driver’s seat, ensuring that all your needs are met. To read more about ISP’s, click here.

How do I know if I am eligible for support?
To find out if you are eligible for an ISP package, or any other services provided by the Tasmanian Governments Department of Health and Human Services Disability Services, you will first need to contact the Gateway Services for an assessment.
Gateway Services is the single entry point to all family and disability services in each area of the State of Tasmania – South East, South West, North and North West. Anyone can talk to the Gateway Services including the person needing assistance, their family and/or carers, specialists, doctors, and other community members. The number to call is 1800 171 233, this number will put you in contact with a Gateway Intake Worker.
If you would like to visit one of the Gateway Services offices, they are located at:
- North Western Tasmania – 56-58 Oldaker Street, Devonport
- South Eastern Tasmania – Level 1/ 10 Bayfield Street, Rosny Park
- Northern Tasmania – Level 1, 8 Boland Street, Launceston
- South Western Tasmania – Ground Floor, 175 Collins Street, Hobart
Can I still get support, even if I am not eligible for government funded services?
Yes, you can!
If government funding is not available or extra support is required, CBS offers self-funded support services.
