Assessment Services

Assessment of a person’s conditions and care needs is an important part of determining the balance of support and independent living that is best for achieving their short and long term goals. At CBS, we know that you understand your needs better than anyone, so we work closely with you during the assessment process to develop a care plan that is individually tailored to you.

Bed and Breakfast Respite

At times, people find that they may need overnight, bed and breakfast style accommodation which is in close proximity to hospitals or other medical professionals to attend appointments etc. CBS has 3 houses which are fully accessibly to people with disabilities and/or people who are frail aged, and/or their carers. Carer support service

Community Based Support (CBS) operates The Commonwealth Respite and Carelink Centre (CRCC) in Southern Tasmania. CRCC provides a range of short term and emergency services to assist carers in their caring role.

Care Coordination

CBS, in partnership with Tasmania Medicare Local, offers case management for people with complex health needs. CBS engages with and supports clients to take a proactive role in the ongoing management of their chronic condition. The role of the Care Coordinator is to liaise with the client, carer, GP, and other health care providers or service/education providers as appropriate to create a comprehensive, integrated careplan for the client. Counselling for carers

One on one, or family/group counselling can be arranged through CBS. For more information about this service, please call CBS on (03) 6208 6600, and speak to a coordinator about how we might help.

Domestic assistance

Helps you to remain independent doing the things you enjoy best, by assisting you with light household duties such as vacuuming, cleaning, laundry and ironing.

Education for Carers

CBS offers educational style events for carers to support them in their caring role. Events range from 1 hour sessions to short break ‘getaways’, all designed to offer carers the opportunity to have a break from their caring role whilst meeting other carers and reducing the social isolation that sometimes comes with being a carer. Events also arm carers with the information and training they need to continue to care for both their loved one, and themselves.

Garden maintenance

Up to three hours of garden maintenance each year is available to eligible people, and can include things such as mowing, weeding, general gardening and rubbish removal.

Group Sessions - Social Activities

These programs are aimed at meeting individual needs for socialisation by offering group outings and get togethers that are fun and entertaining, enhancing your quality of life, emotionally, physically and mentally. They assist in reducing social isolation and assist in connecting you to your community.

Home maintenance

Home maintenance covers a wide range of small repair and maintenance tasks in the home including things such as repair and replacement of taps and tap washers, repair and replacement of door and window locks, minor repair of doors, furniture and walls, installation of smoke alarms and installation of key safes.

Holiday support packages

CBS offers a range of support options for people holidaying within Australia. If you are planning a holiday, and would like support for yourself or the person you care for, please contact CBS directly on (03) 6208 6600.

Home modifications

The Home Modification program focuses on the your most immediate needs as they relate to improving safety and accessibility in your home. Home Modifications range from requests such as bathroom reconfigurations and large ramps, to installing safety rails or changing light bulbs.

In-home respite

The In-home Respite Program is designed to give carers a break by providing In-home personal, domestic and social support for the care recipient. This includes things such as walks, cards games, meal preparation, going for a drive, light household tasks and assistance with personal care.

Nursing Support

CBS arranges nursing support on a needs basis in conjunction with our contract partners throughout Tasmania. Please contact CBS directly on (03) 6208 6600 to speak to a coordinator about your support requirements.

Overnight Support

CBS may provide overnight support, at a reduced cost, where the support worker experiences no more than two interruptions to their sleep. For further information, please contact CBS directly on (03) 6208 6600.

Packages of care

Home Care Packages are designed to provide individually planned and coordinated support to meet the daily care needs of older people who are living in their own home.

Personal Care

The Personal Care Program provides a range of personal services including dressing, showering, mobility assistance, medication prompting and meals – which help older people and people with disability to remain at home, leading independent lives.

Referrals to other services

CBS can assist you to find the right service to match your needs. If you require assistance to find the right service to meet your individual needs, please call and speak to a coordinator on (03) 6208 6600.

Residential respite booking service

The residential respite booking service allows individuals to make one call for bookings and receive advice and support when accessing residential respite. It provides a short-term stay in an approved aged care facility for people living in the community who need a break. CBS manages the residential respite booking service in southern Tasmania.

Safety monitoring systems

In conjunction with Tunstall Healthcare, CBS can provide solutions to increase the safety for people living independently in their own home. These can include devices such as personal medical alarms, fall detectors, help buttons, smoke, gas and flood detectors and key safes. Short-term and emergency support

CBS operates the Commonwealth Respite and Carelink Centre (CRCC) in southern Tasmania. The CRCC provides support services for carers of frail older people, people with mental illness and people with disabilities.

Social support

The Social Support Program assists in meeting your needs for socialisation and includes individual activities such as visiting family and friends, shopping and bill paying.

Spring cleaning

This program provides annual spring cleaning to eligible clients; including cleaning of kitchens, bathrooms, toilets, light fittings, skirtings, windows, gutters, stoves, venetian blinds and high dusting. Turning of mattresses is also included.

Social clubs

Social Clubs bring fun, companionship and entertainment to people’s lives. Activities include art and craft, lunch groups, cooking, nature appreciation, gardening, problem-solving and day trips.

Mental Health Carer Support

CBS can assist you to make the transition home after a stay in an acute facility such as hospital. We understand that going home can sometimes present problems for some people, so we offer practical and emotional support services, which are arranged jointly with you and tailored to your needs. Mental Health Carer Support fact Sheet

24 Hour support

This support may be available in situations where the person being cared for does not require intensive support but should not be left alone. Support workers will take meals with the client and have a full night’s sleep with no more than two interruptions. There is a reduced cost per hour for these situations.
